$242.00 – Total fee to process a 2-year License, and an optional wall certificate ($25.00 fee). $217.00 – Total fee to process a 2-year License (without optional wall certificate). $2.50 – Convenience fee for online application. A money order or a cashier check only will be accepted for payment payable to the AZ MASSAGE BOARD. Page #4 of the application must be notarized correctly. List A and B are included with application. (this must be done at a fingerprint location). 
The card should have APPLICANT on the left hand corner. Provide a fingerprint card with your application.Request transcripts from NHI and send directly to the Arizona Massage Board.
If Massage Therapist does have a CAMTC or other State Certification/Licensure, complete the reciprocity application. If Massage Therapist does not have CAMTC or other State Certification/Licensure, complete this online application. Pass the National Certification Board for Therapeutic Massage and Bodywork. Complete MBLEx Result Transfer Form to have proof of test sent to Arizona (if you have already passed MBLEx) OR. 
Pass the MBLEx Exam (have results sent to Arizona) OR.
